SUMMARY:2026 Quality Improvement Summit
DESCRIPTION:Since 2018\, the Institute for High Quality Care (IHQC)\, in partnership with the Community Clinic Association of Los Angeles County (CCALAC)\, has hosted an annual Quality Improvement Summit. This one-day conference is a unique learning event for community clinics\, public healthcare delivery systems\, and other organizations in the safety net to build their knowledge base in quality and process improvement and to make vital connections with each other. \nIn light of the environmental shifts necessary for COVID-19\, IHQC and CCALAC created the QI Summit Series in August 2020\, a series of interactive webinars featuring topics relevant to safety net healthcare during a time of tremendous change. The Summit Series then continued in August 2021\, featuring a variety of panelists from safety net clinics and delivery systems\, subject matter experts\, and representatives from health plans. In August 2022\, taking into account feedback from attendees of the Summit Series\, IHQC and CCALAC hosted the 2022 QI Summit as a single-day virtual conference\, with new networking technology and a packed schedule of panelists. We have since returned to an in-person format and are happy to announce the 2026 QI Summit\, taking place on September 18\, 2026 in Long Beach\, CA! \n

DESCRIPTION:Shaping the Future of Aging & Cognitive Health: Aligning Care\, Community\, and Partnerships \nThis Coffee Connections session will bring together health systems\, community-based organizations\, and academic partners to explore what caring for our aging population looks like in Orange County. \nWhile aging is a broad and complex topic\, this conversation will focus more specifically on cognitive and brain health\, prevention\, and continuity of care—key areas that shape how older adults remain healthy\, independent\, and supported in their communities. \nParticipants will explore what aging in place requires in practice\, including access to local resources \, affordability\, and coordinated systems of support. The conversation will lift up what is working locally\, where gaps remain\, and how to strengthen collaboration across sectors to better support older adults without working in silos. \nThe session will also explore how community and social systems can better support individuals and families in taking action earlier and more effectively\, and will create space for cross-sector and intergenerational perspectives on how we support aging well in our community. Lastly\, the session will touch on the role of research- particularly the importance of including diverse populations in research and ensuring that insights are translated into real-world practice to improve care and outcomes. \nParticipants will leave with practical ideas\, local insights\, and opportunities for connection to support ongoing work across Orange County.
