PROGRAM OVERVIEW
The Care and Coverage Program ensures individuals and families have access to health insurance and the support they need to navigate the healthcare system. The program provides outreach, education, and enrollment assistance for Medi-Cal, Covered California, and other public coverage options. It plays a critical role in reducing healthcare access barriers, promoting continuity of care, and improving health outcomes for vulnerable populations.
OBJECTIVES
- Reduce disparities by focusing on outreach to underserved populations and those experiencing access challenges.
- Educate communities on the importance of maintaining health coverage and utilizing available services effectively.
- Increase enrollment and retention in Medi-Cal, Covered California, CalFresh and other public supported programs.
- Provide health navigation services to ensure patients establish a medical home.
PROGRAM IMPACT
The Care and Coverage Program directly supports the Coalition’s mission to improve access to healthcare by connecting patients with the coverage they need. It enhances continuity of care, minimizes gaps in coverage, and strengthens health equity across Orange County. By addressing coverage barriers and guiding patients through the healthcare system, the program contributes to better patient outcomes and aligns with the Coalition’s efforts in value-based care and population health initiatives.
