The primary purpose of the Quality Management Department is to support health center members in delivering high-quality healthcare services. We exist to provide resources, educational opportunities, and facilitate collaborations that enhance the performance of healthcare processes and outcomes. Our mission is to empower health centers by implementing evidence-based practices and fostering a culture of continuous improvement. CORE SERVICES

The Quality Management Department is responsible for facilitating several key committees and teams, including the Medical Provider Forum, Dental Provider Forum, and Clinical Quality Champions. These councils play a vital role in our department’s daily operations and overall mission.

  • Medical and Dental Provider Forums: These dedicated groups monitor the operational pulse of community health centers, ensuring that the latest health trends, challenges, and successes are accurately reflected in local medical practices and policies. Their collective expertise and oversight are crucial in maintaining the highest standards of healthcare for the County’s residents. Within this collaborative environment, peer support and the exchange of best practices thrive. Participants engage in open dialogue, sharing insights, innovative approaches, and evidence-based strategies. Transparency and collaboration among members enhance the quality of care through collective wisdom.
  • Clinical Quality Champions: This group participates in educational programs and activities to strengthen and develop healthcare Quality Improvement (QI) programs. The Clinical Quality Champion Meeting provides a forum for the exchange of ideas, develop professional relationships among members to facilitate the free exchange of information and solutions to mutual concerns, and collaborate on Continuous Quality Improvement (CQI) activities.

Additionally, the Quality Management Department collaborates with community partners, including the County’s Health Care Agency, Local Oral Health Program, CalOptima Health, Schools & Universities, Californian Primary Care Association, and other organizations. Our department actively participates in meetings, task groups, and forums to ensure a cohesive approach to healthcare quality and improvement.

  • HCCN (Health Center Controlled Network): This program supports the Clinical Quality Champions in their efforts to lead and implement quality improvement initiatives across the organization, fostering a culture of excellence and continuous improvement.
  • CPCA SDoH Peer Network: A network dedicated to addressing social determinants of health (SDoH) by fostering collaboration and sharing best practices among healthcare providers.
  • CPCA Dental Director Peer Network: This network supports dental directors in enhancing oral health services through peer collaboration, education, and the exchange of innovative practices.
  • CPCA Quality Improvement Peer Network: A platform for healthcare professionals to collaborate on quality improvement projects, share insights, and develop strategies to enhance patient care.
  • Local Oral Health Program, OCHCA: This program aims to improve oral health outcomes in the community by providing education, resources, and support to local health centers.
