STRENGTHENING COMMUNITY HEALTH IN ORANGE COUNTY
In August 2016, the Coalition of Orange County Community Health Centers was awarded a federal grant and recognized as a Health Controlled Network (HCCN) by the Health Resource and Service Administration (HRSA). Initially designated for 10 of its 26 members, the HCCN has since grown to include 12 members, with plans for further expansion within Orange County and potentially neighboring counties.
HCCNs serve as a vital framework for uniting Federally Qualified Health Centers (FQHCs) and Look-alikes, enhancing the quality of care and improving health outcomes. By facilitating the effective use of ONC-certified electronic health records (EHR), adopting innovative technology, and fostering health information exchange (HIE), the HCCN aims to achieve meaningful advancements in patient care.
