PATIENTS

Family Health Clinic located at 1725 W. 17th Street, Santa Ana, CA 92706  is closing the following services:

  • The Emergency Dental Clinic closed Tuesday, May 6, 2025
  • The Children’s Clinic and Family PACT Clinic (Planning, Access, Care and Treatment) closes on Monday, June 30, 2025

CalOptima Health members can contact 1-888-587-8088 (TTY 711) or use their new doctor search tool to find alternative care, including clinics, urgent care facilities, and provider offices.

Members previously seen by one of the closing clinics can request their medical records by calling the Office of Custodian of Records at 1-714-834-3536 or visiting 200 W. Santa Ana Blvd., Suite 180, Santa Ana, CA, Monday to Friday 8am to 5pm.

PROVIDERS & COMMUNITY PARTNERS

For Providers and Community Partners needing assistance with available client resources for dental, children’s health and/or FAMILY PACT services, call the Health Referral Line at 1 (800) 564-8448. The Health Referral Line is available Monday through Friday, 8am to 5pm.
